Enclosed is my check in the amount of One Hundred Dollars ($100.00). I understand that this sum is not refundable unless my application is rejected by the Board of Trustees.

I agree to deposit the additional sum of Five Hundred Fifty Dollars ($550.00) with Anderson Hills, Inc. upon being notified of my acceptance as a Regular Member. I understand this total sum of Six Hundred Fifty Dollars ($650.00)(plus Ohio sales tax) shall be retained by Anderson Hills, Inc. as a non-refundable Initiation Fee.

I further understand that, in addition to the above Six Hundred Fifty Dollars ($650.00) Initiation Fee, current dues (plus Ohio sales tax) must be paid in full to Anderson Hills, Inc. before I may use its facilities.

I further state that I have read Section I of Article III of the Code of Regulations of Anderson Hills, Inc. set out on the reverse side of this application form, and that I understand and agree to the provisions of Section I of Article III.

ARTICLE III: Membership

Section 1. Classification (Revised 11/15/01)

A.Membership shall be of three designations, and total membership shall be limited to 335 members. Persons 21 years of age or older, married or single, and meeting the other qualifications for membership as hereinafter provided, may apply for membership. A membership of any person extends the privileges of the Corporation to the member’s spouse and all members of the immediate family. The immediate family shall include unmarried dependent children and parents of the member or member’s spouse who respectively live at the home of the member.
B.

All members who have been accepted for membership, or who have full membership in Anderson Hills, Inc., before March 11, 1963, shall be designated and hold membership as “Charter Members”.

C.

All members who are accepted for, or who shall have applied for membership in Anderson Hills, Inc., subsequent to March 11, 1963, shall be designated and hold membership as “Regular Members”.

D.

The Board of Trustees shall review the amount of the membership fee, including the non-refundable initiation fee, at least once each year and may prospectively but not retroactively, adjust the value of either fee upward or downward as may be necessary on the basis of current business considerations, economic circumstances and sound customary accounting procedures, including the level of Club membership, recreational programs offered, facilities provided and any special assessments, capital improvements, depreciation or appreciation of assets. Notice of such adjustment shall be given in writing by the Board of Trustees to the members at least five (5) days prior to any regular of special meeting. Said adjustment shall be final unless modified by a two-thirds (2/3) vote of the members present in person at such regular or special meeting.

E.

“Surviving Spouse Membership” may be granted to a Charter or Regular Member who is surviving single spouse in a family, which has been a Charter or Regular Member of the Club for a minimum of five (5) continuous full years. The granting of a Surviving Spouse Membership is subject to the approval of the Board of Trustees. The Surviving Spouse Member shall pay fifty percent (50%) of the annual dues of members as established by the Board of Trustees. All Surviving Spouse Memberships shall be granted in excess of the maximum number of memberships permitted by this Code of Regulations. (Revised 11-14-94)

F.

“Senior Couple’s Membership” may be granted to up to fifteen senior couples, with one spouse being age sixty-two (62) or older. A Senior Couple shall pay two thirds (2/3) of the initiation and two-thirds (2/3) of the family membership fee. A Senior Couple shall receive all the rights and responsibilities of a Regular Member. Notwithstanding the preceding sentence, a Senior Couple’s privileges do not extend to any children of the members. The fifteen (15) Senior Couple’s memberships shall be granted in excess of the maximum number of memberships permitted by this Code of Regulations. (Adopted change of 10 to 15 memberships 11-15-01).

G.

“Self-Only Membership” may be granted to up to ten individuals over age twenty-one (21). Self-Only members shall pay one-half (1/2) of the initiation and one half (1/2) of the family membership fee. A self-only member shall receive all the rights and responsibilities of a Regular member. Notwithstanding the preceding sentence, a self-only member’s privileges do not extend to other family members of the self-only member If a Self-Only Member marries and wants to a Regular Membership or desires to convert to a Regular Membership, then this individual will be placed on the “On Hold” list for entry. When a Regular Membership is available, the family will be permitted to enter upon payment of one-half (1/2) of the Initiation Fee and full payment of the regular dues. Regular members converting to Self-Only Membership are not entitled to a refund of the difference in initiations previously paid. The fifteen (15) Self-Only Memberships shall be granted in excess of the maximum number of memberships permitted by this Code of Regulations.
